Among other things the report confirms Hillary Clinton never received authorization (S/ES-IRM, DS), she hid the server from security audits, she did not want her personal emails accessible (FOIA/NARA), and she failed to implement safeguards and controls for archiving records. Her server was the target of attacks, which were never officially reported to S/ES-IRM as required.
The report also revealed, that longtime Bill Clinton aide Justin Cooper, had admin access to Clinton's server. [Cooper was an employee of Teneo Holdings](http://web.archive.org/web/20121223090947/http://teneoholdings.com/our-team/bio/justin-cooper) and The Clinton Foundation and held no security clearances.

Pagliano received immunity from the FBI in the Fall of 2015. Pagliano has come under fire recently as he had no security clearances and lacked adequate infosec/netsec certs and experience. The State Department claimed it can not find any of his emails during his employment between May 2009 - Feb 2013.

In an email dated August 30, 2011, Bentel stated, "We actually have an account previously set up: SSHRC@state[.]gov. There are some old emails but none since Jan '11 - we could get rid of them. You should be aware that any email would go through the Department's infrastructure and subject to FOIA searches."
